Objective:
The Community-Based Program Case Manager assists individuals and families experiencing homelessness or housing instability with obtaining and maintaining safe, stable housing. The Case Manager provides individualized, client-centered case management in clients’ homes and throughout the community, including assessment, goal planning, housing stabilization, connection to community resources, advocacy, and ongoing monitoring of client progress.

 Duties and Responsibilities:
  • Conduct regular case management meetings with assigned clients, generally at least once per week or as required by program guidelines. Visits should occur in the client’s home whenever possible unless circumstances require an alternative location.
  • Work collaboratively with clients to identify needs, strengths, barriers, and priorities and develop individualized, strength-based service and goal plans.
  • Regularly review client goals and service plans, assess progress, and recommend or implement changes when appropriate.
  • Assist clients in locating, securing, and maintaining housing consistent with program requirements and individual needs.
  • Develop and maintain knowledge of housing opportunities, landlords, rental resources, and housing programs throughout Bucks County and surrounding communities.
  • Assist clients with identifying education, employment, job training, income, and benefit opportunities and removing barriers to accessing those resources.
  • Support clients in developing and maintaining education, employment, income, and housing-stability plans through regular communication with clients and, when appropriate, other service providers and supports.
  • Advocate with and for clients to support successful completion of their housing and service plans.
  • Assist clients with developing independent living and housing-stability skills, including budgeting, household management, communication, organization, and other skills necessary to maintain housing.
  • Connect and refer clients to appropriate community resources, including mental health services, substance-use treatment, employment assistance, transportation, health care, benefits, training programs, food resources, and other services based on individual needs.
  • Assist clients with identifying and overcoming barriers that may prevent successful connection to referred services.
  • Collaborate with other BCHG case managers, program staff, landlords, community agencies, and other supports involved with clients.
  • Maintain positive working relationships with Bucks County Children and Youth, Bucks County departments, housing providers, social-service organizations, and other community partners.
  • Maintain complete, accurate, and timely case documentation and data entry through the Homeless Management Information System (HMIS) and any other required program databases.
  • Ensure all program-specific documentation, assessments, service plans, eligibility documentation, and client files are accurate, complete, and maintained in accordance with program and agency requirements.
  • Maintain required program outcomes and assist with the collection and reporting of outcome data.
  • Complete required program billing and financial documentation accurately and on time, including monitoring client rental obligations or payments when applicable to the program.
  • Meet regularly with the Social Services Manager and other case management staff to review client progress, program performance, challenges, and service coordination.
  • Participate in staff meetings, trainings, case conferences, and other agency activities as required.
  • Complete other assignments as determined by the Social Services Manager or Executive Director, including assisting clients participating in other BCHG programs when necessary.



Essential Skills and Knowledge:
  • Client-Centered Approach: Ability to work effectively with individuals and families from diverse backgrounds using a respectful, empathetic, trauma-informed, and non-judgmental approach.
  • Case Management and Social Work Knowledge: Working knowledge of case management and social work principles, methodologies, professional standards, boundaries, and ethical practices.
  • Problem-Solving: Ability to identify barriers, analyze complex client situations, develop practical solutions, and respond appropriately to changing circumstances.
  • Communication: Strong oral and written communication and interviewing skills and the ability to communicate effectively with clients, landlords, community partners, coworkers, and other stakeholders.
  • Organization and Documentation: Strong administrative, organizational, and time-management skills, including the ability to maintain accurate case documentation, meet deadlines, and manage multiple client and program responsibilities simultaneously.
  • Technology: Ability to effectively use Microsoft Office, HMIS, and other database or case-management systems required by the program.
  • Community Resource Knowledge: Knowledge of Bucks County housing, homeless services, public benefits, behavioral health, employment, transportation, and other community resources, or demonstrated ability and willingness to quickly develop this knowledge.
  • Conflict Management: Ability to effectively manage difficult conversations, interpersonal conflict, and challenging client situations while maintaining appropriate professional boundaries.
  • Independent Work: Ability to work independently in community settings while maintaining effective communication and coordination with supervisors and team members.
  • Collaboration: Ability and willingness to collaborate with BCHG staff, community agencies, landlords, government partners, and other stakeholders to develop solutions to complex client needs.
  • Cultural Responsiveness: Sensitivity to and understanding of the needs of individuals and families from diverse cultural, socioeconomic, and personal backgrounds.
  • Caseload Management: Ability to effectively organize, prioritize, and manage an active community-based caseload.
  • Transportation: Must have access to a reliable vehicle, possess a valid driver’s license, and maintain appropriate automobile insurance. Frequent travel throughout Bucks County is required, and occasional travel outside Bucks County may be necessary.
  • Physical Capability: Ability to perform the routine physical requirements associated with community-based case management, including traveling to client residences and community locations and occasionally lifting, pushing, pulling, or carrying objects weighing up to 25 pounds.

Education and Professional Qualifications:
Preferred:
Bachelor’s degree in Social Work, Human Services, Psychology, Sociology, or a related field and at least two years of case management or related human-services experience;
OR
Associate’s degree in Social Work, Human Services, or a related field and at least four years of case management or related human-services experience.
Preference will be given to candidates with prior experience working with individuals or families experiencing homelessness or housing instability and familiarity with housing programs, homeless services, HMIS, and community-based service delivery.

Additional Details: 
This is a full-time, non-exempt position reporting directly to the Social Services Manager and working cooperatively with the BCHG Social Services Department.
This is a community-based position requiring regular travel to clients’ homes, community agencies, housing locations, and other locations throughout Bucks County. Administrative work may be completed from BCHG offices and other approved locations as appropriate to program and agency needs.
Candidates must successfully complete all background checks and clearances required by BCHG and applicable programs, including Pennsylvania criminal background and child abuse clearances.
Introductory Period: 90 days
Compensation and Benefits
Salary is $46,000 - $48,000 commensurate with experience.
